
We can help young adults who are moving from children’s services to adult services to develop their independence and to have choice and control over the support they need to achieve their hopes and dreams. These clients may be young adults from the local authorities, asylum seekers, or children leaving the care system to live independently. We plan services around each individual person and their needs.
These includes:
- Help to find accommodation that meets the needs of each individual.
- Supporting the young person to develop new skills in order to be more independent.
- Supporting the young person to engage with their local community and enjoy all it has to offer.
- We encourage people to enjoy family life, make friends and lead a full life.
- Ensuring that the people we support are safe and happy.
- Providing person-centred support, working in partnership with individuals, their families and healthcare professionals.
- Working with the individual and their families to select their own support staff.
- Supporting the young person to find employment.
- We can help any individual approaching the age of 18 who has been assessed as requiring care and who would like us to start planning their future support.
